COVID-19 VACCINATION REGISTRATION

COVID-19 Vaccine Registration Button Opens in new window

COVID-19 vaccines are available to everyone age 12 and over! The three COVID-19 vaccines authorized for use in the US (Pfizer, Moderna and J&J) have been rigorously tested and shown to be highly safe and highly effective in both laboratory and real world settings. To date, more than half a million Montco residents have received at least one dose of a COVID-19 vaccine while over 3.6 billion doses have been administered worldwide.

Getting vaccinated can help keep your family, friends, community – and yourself – safe. It’s also the best way to end the pandemic and ensure a lasting return to normalcy.

VACCINE SUPPORT FOR PEOPLE WITH DISABILITIESGetting vaccinated as a minor in montco

All minors (under age 18) seeking to get vaccinated at a Montgomery County Office of Public Health-run clinics, MUST either be physically accompanied by a parent/legal guardian or an authorized substitute. Authorized substitutes are adult relatives, age 21 or older, who have been authorized by the child's parent to seek medical treatment for the minor.  This may include grandparents, aunts/uncles, or siblings (provided they are at least 21 years old). The authorized relative must present a signed copy of the Treatment of a Minor form. If the authorized relative does not have a signed form, the parent may be contacted by phone to give a verbal consent for delegation of care to that minor, and the form should be completed with the parent signature line indicating “verbal consent” and the name of the person witnessing consent.

Please note: Only the Pfizer vaccine has been authorized for those under the age of 18.
