Connect with FREE Montgomery County Family Supports in the Pottstown Area!
Pregnant women and families with children under age six can benefit from evidence-based family support programs shown to improve maternal and child health, prevent child abuse and neglect, and improve family literacy and economic security.
In 2015, the Maternal and Early Childhood Consortium (MECC) of Montgomery County initiated conversations to coordinate family support services in the county. In August 2018, five Home Visiting programs started a pilot Coordinated Referral System in Pottstown. These agencies include Maternity Care Coalition’s Early Head Start, Family Services’ Parents as Teachers, MCH Consortium of Chester County’s Parents as Teachers, Einstein Hospital’s Nurse-Family Partnership, and Montgomery County’s Partners for a Healthy Baby.
Our goal is to make it easy for you to connect with the family supports that you desire. Fill out one referral form and submit it via fax, phone or e-mail below, and we will connect you with the program that seems to be the best fit. If you have a program preference, please include it on the referral and we will connect you with the agency of your choice.
