For Providers/Community GRoups
One of our goals is to engage with residents in areas of the county where transportation may be challenging, people feel isolated, or are unaware of the Community Connections Program and the Department of Health & Human Services. In order to meet this goal, Community Connections Navicates expand their reach throughout the County by spending a few hours per month at partner organizations who frequently see residents in need of various services beyond what their organization or agency may provide, for example food pantries or local libraries. While on-site, a Navicate will educate staff and residents regarding the roles of a Navicate and will work directly with residents to identify and provide additional resources; if necessary, they will advocate on their behalf to remove any barriers in accessing those resources.
How Does My Site Get Added to this List?
These sites are not intended to be permanent locations for Navicates. As such, we are always searching for new places to connect with residents and organizations unfamiliar with our services and will explore all options for new Expansion Sites. If you have an idea or are interested in learning if your site can be piloted, please click here to submit your information and request.
